POSITION:0.4 FTE Mid-Level Provider - Health Services

REPORTS TO:           Director of Nursing & Clinical Services

PRIMARY FUNCTIONS:  Performs health assessments and risk appraisals on students within the Hartford Public School System.

SKILLS, KNOWLEDGE, QUALIFICATIONS AND EXPERIENCE:

·      Must be a graduate of an of an accredited APRN/PA program.

·      Must hold a current APRN/PA Connecticut State license.

·      Must have a desire to work with school-aged children.

·      Must be knowledgeable of school health procedures, including state and Federal mandates.

·      Successful experience as a school practitioner in previous position is desirable.

·      Must have own transportation.

·      Bilingual (English/Spanish) preferred.

CERTIFICATION REQUIREMENTS:

Current CPR certification required.

Must have a certification of completion from an approved program for APRN/Pat.

SALARY AND TERMS OF EMPLOYMENT:

10 month work year (187 days). Salary in accordance with Hartford Federation of School Health Professionals Appendix A, 5C ($28,148.40-$39,144.80) 0.4 FTE Equivalency.

UNION AFFILIATION:

Hartford Federation of School Health Professionals (HFSHP).
