Description
The Recovery Specialist is responsible for assisting the Program Coordinator and Behavioral Health Specialist in the delivery of direct services to program guests, from the perspective of the person he/she is serving. The Recovery Specialist has experience receiving mental health services and provides a variety of services, ranging from the teaching of skills to delivery of concrete services to providing individual and group counseling to the program guests to providing outreach, engagement and program tours.
Duties and Responsibilities:
Provides direct care services to program guests including linkages to community resources and education on alternative methods of coping skills that can be utilized during acute crises to reduce hospitalizations.
Complete documentation such as Pre-Registration Questionnaire, Individual Service Plan, Functional Assessment, Discharge Plan, and Follow-up documentation. Daily meetings with guests assist with development of coping skills and review progress. Responsible for all documentation required for program guest record (e.g., progress notes, collateral contacts etc.).
Converse with various shifts regarding any pertinent information (i.e. appointments, medication changes, overall guests’ behaviors, etc.) in communication log.
Provide recovery readiness services in groups and individual counseling. Facilitate support, therapeutic and educational groups for residents.
Periodic room inspections for safety measures and to report any concerns, damages or problems to shift supervisor. Assist guests with the upkeep & cleanliness of room, as needed.
Educates and assures that consumers know their rights and responsibilities and assists consumers with identifying their options and participating in all decisions related to establishing and achieving goals. Educate staff about peer perspective, person-centered language, rights and protections, and alternatives
Provides crisis intervention, and rehabilitative support services in daily living skills, health services and others tasks as assigned.
Monitors the site, coordinates and participates group activities such as recreational activities, safety checks, activities of daily living etc.
Participate in program meetings, staff meetings and attend supervision regularly. Attend trainings provided by agency and outside sources as required.
Accompany program guests to appointments if needed to provide additional support and understanding.
Flexibility in scheduling according to program needs.
Adherence to all policies and procedures as set forth in Concern’s employee manual.
Remain current with all Agency required compliance documents and trainings.
Completes other duties as assigned by the Program Coordinator/Behavioral Health Specialist.
Salary: $20.00/hr. for 30 hour position and $41,600/yr. for the full-time position
Position Requirements
Must have High School Diploma and have completed a peer specialist or peer wellness training within the last 3 years. High School Diploma required. Valid license required. Computer skills required. The individual in this position must possess the ability to communicate effectively, guide others through the self-help process and make sound judgments regarding care. Experience with travel training, facilitating peer support & educational groups and/or assisting others with ADL skills required.
