The Bulloch County Wellness Clinic is seeking a highly qualified candidate to fill the full-time, salaried position of RN Case Manger 1, 2, or 3  based in Statesboro, GA

As a RN Case Manager with the Bulloch County Wellness Clinic, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.

Job Responsibilities

Responsible for HIV Case Management services that include support, care, and treatment of HIV/AIDS clients and/or their families.  Case Management also includes, but is not limited to, patient assessments, medical referrals and social support assistance.  As a member of the clinic team, will be responsible for assuming expanded functions requiring specialized nursing knowledge, skills, and judgment in the provision of services to clients in the Infectious Disease Program with a focus in HIV/AIDS.  Will develop and implement an appropriate plan of care based on assessments.   Help clients with medication application process to ensure clients have meds when needed.  Also, assists with co-pay cards and patient assistance programs.   Fast-paced environment that requires multi-tasking and great time-management skills. Will also assist with Nursing coverage for lab when needed.

Minimum Qualifications

Associate's degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification.  Note: No experience required.

Preferred Qualifications:

Completion of a bachelor’s degree in nursing.  One year of experience as a registered professional nurse. Experience working with HIV/AIDS patients.
