Duties and Responsibilities Include but not limited to:<ul><li>Facilitate residents linkage with services available in the community (e.g., case management, clinical services, day programs, vocational services and opportunities, community integration, entitlements, etc.).</li><li>Development of comprehensive needs assessments and support plan. Meetings with clients to develop skills and review progress. Responsible for all documentation required for client record (e.g., progress notes, discharge plans, etc.).</li><li>Provide individual counseling and support services.</li><li>Provide medication management services</li><li>Engage in recovery planning through the use of Wellness, Recovery and Action Planning</li><li>Provide daily living skills training (e.g., medication monitoring and training, cleaning, money management, etc.).</li><li>Attendance at supervision and staff meetings.</li><li>Remain current with all Agency required compliance documents and trainings.</li><li>Other duties as assigned by the Program Supervisor.</li></ul> Salary: $50,000/year&nbsp; Position Requirements Bachelor’s Degree preferred in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 2 years’ experience working in a similar setting and/or working with people with psychiatric disabilities.&nbsp;The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
